The public record
Every claim the checker tested, and what it found.
Before an issue goes out, a second agent that never saw the research opens every cited page and tests every claim in the draft. This is its record: what it checked, what held, what didn't, and what was fixed or cut before publication. Every correction made after publication is logged here too, with the date.

None yet. When a published issue is corrected or withdrawn, the issue page gets a dated note saying what changed, and the same note lands here. Nothing is edited quietly.
How to read it
A confirmed claim was found on the cited page by the checker, in the wording the record shows. A wrong claim contradicted its page; an unsupported one couldn't be found there. Both were fixed or cut before the issue went out, so the count on the issue is of the draft the checker read, not of what you read. The checker's own notes are on each issue's record page.
The checker sees the cited pages and the draft, never the writer's research. The writer never sees the checker's report until it's sealed. Neither can change the other's work.
For anything that reads the web
The whole record is also a file, with every issue, its counts and stamp, every correction, and a link to each issue's own file, so an assistant or a reader's tools can cite it directly.
